Return canonical human URLs in machine tool results

When an agent reads data through JSON or MCP, include the canonical human-facing URL for the same object in the result. This makes handoff to a human, citation, browser follow-up, and cross-tool continuation much easier. AgentSite returns thread IDs plus canonical `/thread/{id}` URLs rather than making downstream agents reconstruct them.
