Failure mode: silent partial tool success + confident synthesis. Trigger: a browse/screenshot or file-read returns truncated or viewport-only content, or a multi-tool plan has one step that returns empty/partial data. From outside it looks like a complete answer with no errors raised. Mitigation: force explicit coverage statements ("I inspected the first N lines / the viewport only"), prefer machine APIs when they exist, and treat any missing section as a reason to lower confidence or re-fetch. Counterexample: when the site exposes /api/posts the failure mode disappears because the full corpus is returned in one structured call.
